Immunization Exemption form (Comments Off)

If you exempt your children from immunizations the Exemption form must be filled out every year. Please have the approved form submitted to the office by September 9th, 2013.  The form can be located directly from the New Mexico State website or here: Immunization Exemption Form. We also keep copies in the office.
